The Basics

For yourself—and for anyone for whom you are the Executor

Where are these kept? Collect copies of these documents into one place, such as a notebook or file folder. Also, scan them into your computer files.
  • Social Security Card
  • Driver’s license
  • Birth Certificate
  • Last Will and Testament*
  • Living Will/Advance Directive*
  • Power of Attorney*
  • Trust Agreement(s)*
  • List of current medications (carry in wallet also)
  • In Case of Emergency (ICE) contact information. Enter in cell phone. Also register this with the Bureau of Motor Vehicles
  • Military Discharge papers
* The starred items must be completed when all parties are competent. If you wait until you are in crisis, you may not be able to complete these. Plan ahead. Two a.m. in the Emergency Room is not the time to start this process.

Remember, you need to know where your parents’ information is, also.
